Social Work Services
Our licensed medical social workers are available to assess your
needs and assist you with social, psychological, financial, and
hospital discharge planning issues. They work closely with you,
your family, your physicians and case managers at St. Mary's to
ensure that you have an appropriate plan of care upon your
discharge. Our social workers provide counseling in relation to
coping with your illness, as well as crisis intervention, planning
and arranging for short-term rehabilitation programs and long-term
care. In addition, they are available to assist you in the
completion of an advance directive and other decision-making
processes relating to end of life issues. To request a meeting with
